Habitat Health with Kaiser Permanente opens its first innovative PACE health care program for older adults in Sacramento

The partnership expands access to comprehensive care for older adults who wish to remain in their homes and communities.

SACRAMENTO, Calif., Jan. 2, 2025 /PRNewswire/ -- Habitat Health is proud to announce the opening of its first Program of All-Inclusive Care for the Elderly (PACE) center in partnership with Kaiser Permanente, expanding access to vital health care services for older adults. At the new center, located at 855 Howe Avenue in Sacramento, Habitat Health with Kaiser Permanente offers participants a fully integrated health care model that includes primary and specialty care, prescriptions, social activities, transportation, home care, and more, all coordinated under one roof.

Habitat Health provides comprehensive medical and social care to eligible older adults to help them maintain their independence and stay in their homes and communities. Through the partnership, Habitat Health with Kaiser Permanente PACE participants have access to Kaiser Permanente's industry-leading clinical expertise and sophisticated network of health care services. These services are typically provided at no cost to program participants who are eligible for Medicare and Medi-Cal coverage.
